Flexible per diem opportunity available for a Nurse Practitioner with experience in home health to conduct comprehensive risk assessments in Miami-Dade County. This position allows you to set your own schedulechoose shifts between Monday and Sunday, and determine your preferred weekly or bi-weekly hourly availability. Assessments are typically completed in about 45 minutes, providing a concise yet impactful interaction with Medicare Advantage members in various neighborhoods across Miami, Hialeah, and Miami Gardens. Assignments may include annual health evaluations or post-discharge visits based on member needs.
Key Responsibilities:
- Complete home health risk assessments for Medicare Advantage plan members
- Review medical, surgical, and medication histories with each patient
- Deliver education on disease management and medication adherence
- Identify gaps in care and recommend next steps to enhance patient health outcomes
- No responsibility for prescribing medications or developing treatment plans
Qualifications:
- Active Nurse Practitioner license in Florida
- Board Certification through either AANP or ANCC
- Current CPR (BLS) certification
- MSN from an accredited school of nursing
- At least 6 months of recent Nurse Practitioner experience preferred
This role offers compensation of $80$90 per evaluation, in addition to a mileage stipend and reimbursement for required administrative time.
