Take on a vital role as a Nurse Practitioner delivering home health risk assessments to Medicare Advantage members throughout Miami-Dade County. This per diem opportunity offers flexibility and autonomy, allowing you to design your own schedule with shifts available any day of the weekincluding weekends. Assignments may include locations such as Miami, Hialeah, and Miami Gardens, where you'll meet with patients in their homes to support their ongoing health management.
Schedule:
- Fully flexible shifts between Monday and Sunday
- Set your own hours and workload
- Minimum commitment required on a weekly or bi-weekly basis
Key Responsibilities:
- Perform comprehensive annual home health risk assessments for Medicare Advantage plan members
- Review and evaluate prior and current medical history, health conditions, and medication usage
- Educate patients on their medical conditions and prescribed medications
- Identify care gaps and offer practical recommendations to optimize patient well-being
- Focus on assessment and educationno treatment planning or prescribing necessary
Qualifications:
- Active Nurse Practitioner license in Florida
- Current BLS CPR certification
- Board certified by AANP or ANCC
- MSN degree from an accredited nursing program
- At least 6 months of recent nurse practitioner experience is preferred
Youll earn $80$90 per completed evaluation, plus a mileage stipend and compensation for administrative time.
