Job Description

Enjoy the flexibility of per diem work as a dedicated Nurse Practitioner providing home health risk assessments within Victoria County, TX. In this dynamic role, you will be responsible for evaluating Medicare Advantage plan members in their homes, supporting their health and well-being through comprehensive risk assessments. Shifts can be scheduled across Victoria, Bloomington, and other nearby areas, tailored to your availability.

Schedule:

  • Flexible shifts offered Monday through Sunday
  • Design your own work schedule
  • Minimum commitment based on weekly or bi-weekly hours

Key Responsibilities:

  • Perform annual home health risk assessments for Medicare Advantage members
  • Review and document prior and current medical history, ongoing conditions, and medication usage
  • Deliver clear education to members on their medical conditions and prescriptions
  • Identify care gaps and provide actionable recommendations to improve patient outcomes
  • Visits are strictly for assessmentno prescribing or treatment plan creation required

Qualifications:

  • Active Nurse Practitioner license in Texas
  • Board Certification through AANP or ANCC
  • Current CPR (BLS) certification
  • MSN degree from an accredited nursing program
  • At least 6 months of recent experience practicing as a Nurse Practitioner preferred

This position offers a competitive per-evaluation rate of $80-$90, plus additional compensation for mileage and administrative duties.
