Play a pivotal role in supporting community health by performing home health risk assessments for Medicare Advantage members throughout Oswego County. This per diem opportunity offers a flexible schedule that empowers you to tailor your work hours between Monday and Sunday, with options to commit on a weekly or bi-weekly basis. Assignments are located in various parts of Oswego County, including Mexico, Fulton, Brewerton, Pulaski, and nearby areas. Each visit will last approximately 45 minutes and may involve either an annual assessment or a post-discharge review.
Schedule:
- Flexible shifts available any day of the week

- Minimum weekly or bi-weekly hours as you prefer
Key Responsibilities:
- Conduct comprehensive annual home health risk assessments for Medicare Advantage recipients
- Review patients previous and current medical conditions, as well as medication histories
- Educate patients on diagnoses and medication management
- Identify care gaps and make recommendations to address them
- No treatment plan creation or medication prescribing required
Qualifications:
- Active Nurse Practitioner license in New York
- MSN from an accredited nursing school
- Board Certification via AANP or ANCC
- Current CPR (BLS) certification
- At least six months of current NP experience preferred
This role offers competitive earnings of $80$90 per evaluation, along with a mileage stipend and compensation for administrative time.
