Advance your Nurse Practitioner career by conducting home health assessments in Oswego County, NY, as part of a dynamic and flexible per diem opportunity. In this role, youll assess Medicare Advantage members residing in communities such as Fulton, Brewerton, and Pulaski. Your visitseach averaging 45 minutesmay include annual health assessments or post-discharge check-ins, ensuring a comprehensive review of each patients health status in the comfort of their own home.
Schedule:
- Flexible shifts scheduled Monday through Sunday
- Self-directed, create-your-own schedule
- Commitment to minimum weekly or bi-weekly hours
Key Responsibilities:
- Perform annual home health risk assessments for Medicare Advantage members
- Review and evaluate medical history, current conditions, and medication usage
- Educate patients on their medical conditions and prescribed medications
- Identify gaps in care and make recommendations for follow-up
- No requirement for developing treatment plans or prescribing medications
Qualifications:
- Active Nurse Practitioner license issued by NY State
- Current CPR certification (BLS required)
- Board certified by AANP or ANCC
- MSN from an accredited nursing program
- At least 6 months of experience practicing as a Nurse Practitioner preferred
