Deliver high-quality care directly where patients need it most by performing home health risk assessments for Medicare Advantage plan members throughout Josephine County. This per diem nurse practitioner role offers flexibility and an impactful scope of practice without the responsibility for ongoing treatment or medication management. Work with individuals in various locations, including Grants Pass, Golden, Greenback, Waldo, and Cave Junction, while managing your own schedule.

Schedule & Flexibility:

• Flexible shifts available Monday through Sunday
• Self-scheduling to suit personal and professional commitments
• Minimum weekly or bi-weekly hourly commitment required

Key Responsibilities:

• Conduct comprehensive annual home health risk assessments in patients' residences
• Review prior and current medical history, chronic conditions, and medication regimens
• Educate patients on managing their health conditions and proper medication use
• Identify potential gaps in care, offering recommendations to improve patient outcomes
• Visits may include annual health assessments or post-discharge evaluations
• No prescribing medications or developing treatment plans

Qualifications:

• Active Nurse Practitioner license in Oregon
• Board Certification through AANP or ANCC
• Current CPR (BLS) certification
• 6+ months of current nurse practitioner experience preferred
• MSN degree from an accredited nursing program

This opportunity provides competitive per-assessment pay between $80 and $90, plus a mileage stipend and compensation for administrative time.
