Job Description

Step into a rewarding role conducting home health risk assessments in Hartford County, CT. As a Nurse Practitioner specializing in home health, you will engage with Medicare Advantage plan members across areas such as Hartford, New Britain, and West Hartford. This per diem opportunity allows practitioners to create their own schedules, with flexible shifts available seven days a week and a commitment that adjusts to your availability.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Build your own work schedule
  • Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

  • Complete annual and post-discharge risk assessments in members homes
  • Review and document prior and current medical histories, ongoing conditions, and medication regimens
  • Educate patients on their health conditions and prescribed medications
  • Identify gaps in care, making recommendations to promote optimal health outcomes
  • No requirement to prescribe medications or develop treatment plansfocus is on assessment and education

Qualifications:

  • Active Nurse Practitioner license in the state of Connecticut
  • Board-certified through AANP or ANCC
  • Valid CPR (BLS) certification
  • MSN degree from an accredited nursing program
  • At least 6 months of Nurse Practitioner experience preferred
