Were seeking a Nurse Practitioner to perform home health risk assessments in various communities across Adams County, IL. Schedule:
- Flexible shifts available Monday through Sunday
- Design your own work schedule
- Minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Perform comprehensive home health risk assessments in the patients residence
- Review and evaluate prior and current medical history, existing medical conditions, and medication lists
- Offer patient education surrounding their diagnoses and medications
- Identify care gaps and make informed recommendations to improve patient outcomes
- Focus solely on assessmentno prescribing or creation of treatment plans involved
Qualifications:
- Active Nurse Practitioner license in IL
- Current CPR certification (BLS)
- Board Certification through AANP or ANCC
- Minimum of 6 months of NP experience preferred
- Master of Science in Nursing from an accredited program
You will be traveling throughout Adams County, including areas like Quincy, Camp Point, and Payson, providing critical annual health or post-discharge assessments. Each evaluation typically takes about 45 minutes, and you will receive $80-$90 per completed assessment, along with a mileage stipend and compensation for administrative time.
