Advance your career as a Nurse Practitioner specializing in home health by conducting comprehensive risk assessments for Medicare Advantage plan members. In this dynamic per diem role, you will play a vital part in supporting patients throughout Shelby County, providing essential evaluations in their homes and making a meaningful impact on their long-term health outcomes.
Schedule:
- Flexible shifts available between Monday and Sunday
- Ability to create your own schedule
- Minimum weekly or bi-weekly hourly commitment required
Key Responsibilities:
- Conduct detailed annual home health risk assessments in patients homes, typically taking 45 minutes per visit
- Evaluate prior and current medical history, medical conditions, and medication regimens
- Offer patient education on conditions and prescribed medications
- Identify gaps in care and make recommendations to ensure optimal outcomes
- Perform visits that may include annual health assessments or post-discharge evaluations
- Focus exclusively on risk assessmentsno requirement to create treatment plans or prescribe medications
Qualifications:
- Unrestricted Nurse Practitioner license in Tennessee
- Masters degree in Nursing (MSN) from an accredited institution
- Current BLS (CPR) certification
- Board Certification through AANP or ANCC
- At least 6 months of recent Nurse Practitioner experience preferred
Compensation & Benefits: Nurse Practitioners in this role will earn between $80$90 per completed evaluation, with mileage stipend and administrative time compensated separately, allowing you to maximize your earning potential while enjoying flexible, self-determined shifts.
