Schedule:
- Flexible shifts available Monday through Sunday

- Minimum weekly or bi-weekly hourly commitment required
Key Responsibilities:
- Conduct comprehensive annual home health risk assessments for Medicare Advantage plan members
- Review prior and current medical history, chronic conditions, and medication regimens
- Educate patients on managing existing conditions and proper medication use
- Identify care gaps and recommend next steps to improve health and outcomes
- Focus on evaluationno treatment plans or medication prescribing required
Qualifications:
- Licensed Nurse Practitioner (NP) in Tennessee
- Current CPR certification (BLS)
- Board certification through AANP or ANCC
- Six months or more as a Nurse Practitioner preferred
- Master of Science in Nursing (MSN) from an accredited program
This role offers a highly competitive per-evaluation rate between $80 and $90, supplemented by a mileage stipend and compensation for administrative time.
