Job Description

Deliver comprehensive care in the community as a Nurse Practitioner specializing in home health assessments. This per diem role offers maximum flexibility, enabling you to design your own schedule with available shifts any day of the week. You will provide important healthcare services to Medicare Advantage members across various locations in Gibson County, including Humboldt, Milan, and Trenton.

Schedule:

  • Flexible shifts offered Monday through Sunday
  • Opportunity to create your own work schedule
  • Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

  • Conduct thorough annual home health risk assessments for Medicare Advantage plan members in their homes
  • Review and evaluate patient medical and medication histories, including prior and current conditions
  • Educate patients on medical diagnoses and appropriate medication usage
  • Identify gaps in care and advise on strategies for resolution
  • Perform visits that may serve as annual assessments or post-discharge evaluations
  • No requirement for writing treatment plans or prescribing medications

Qualifications:

  • Active Nurse Practitioner license in Tennessee
  • Board Certification through AANP or ANCC
  • Current BLS certification (CPR)
  • MSN degree earned from an accredited nursing school
  • At least 6 months of recent Nurse Practitioner experience preferred

Successful candidates will be compensated $80-$90 per evaluation, with an additional stipend provided for mileage and administrative time.
