Job Description

Advance your clinical expertise while enjoying flexible, per diem work as a Nurse Practitioner specializing in home health risk assessments. In this role, youll perform comprehensive evaluations for Medicare Advantage plan members throughout Summit County, Ohioincluding the communities of Akron, Cuyahoga Falls, Stow, Barberton, and more. Each assessment averages 45 minutes and may include annual health reviews or post-discharge visits, promoting optimal patient outcomes in their home environment.

Schedule/Hours:

  • Flexible shifts available Monday through Sunday
  • Self-scheduling to fit your life and commitments
  • Set your own minimum weekly or bi-weekly hourly targets

Primary Responsibilities:

  • Perform thorough home-based risk assessments for Medicare Advantage members
  • Review patients medical, surgical, and medication histories
  • Educate patients on health conditions and medication regimens
  • Pinpoint gaps in care and provide actionable recommendations
  • No treatment planning or prescribing responsibilities required

Required Qualifications:

  • Active Nurse Practitioner license in Ohio
  • Master of Science in Nursing (MSN) from an accredited program
  • Board Certification through AANP or ANCC
  • Current CPR/BLS certification
  • At least six months of recent Nurse Practitioner experience preferred
