Job Description

Advance your career as a Nurse Practitioner specializing in home health by performing comprehensive in-home assessments for Medicare Advantage plan members in Warren County, TN. This per diem position allows you to create your own schedule with flexible shifts throughout the week. Assignments are available in McMinnville, Morrison, Centertown, and nearby communities, based on the needs of the health plan and your availability.

Schedule:

  • Flexible shifts, Monday through Sunday
  • Self-schedulingchoose shifts that suit your lifestyle
  • Minimum weekly or bi-weekly commitment required

Responsibilities:

  • Conduct 45-minute in-home risk assessments, including annual and post-discharge evaluations
  • Review patients past and current medical histories and medication use
  • Educate patients on management of health conditions and proper medication usage
  • Identify any care deficiencies and offer recommendations to bridge care gaps
  • No responsibilities for creating treatment plans or prescribing medications

Qualifications:

  • Active Nurse Practitioner License in Tennessee
  • Current BLS (CPR) certification
  • Board Certification through AANP or ANCC
  • At least 6 months of recent nurse practitioner experience preferred
  • MSN degree from an accredited nursing program

This role offers competitive compensation of $80-$90 per completed evaluation, along with a mileage stipend and compensation for administrative time.
