An opportunity is available for a skilled Nurse Practitioner to perform home health risk assessments on Medicare Advantage plan members in Warren County, TN. This per diem position is well-suited for professionals seeking schedule flexibility and the chance to make a direct impact through one-on-one patient care in a home health setting. The average assessment lasts approximately 45 minutes and may include annual health evaluations or post-discharge visits. Shifts may be scheduled in McMinnville, Morrison, Centertown, or other areas within Warren County.
Schedule:
- Flexible shifts available seven days a week (MondaySunday)
- Design your own work schedule
- Minimum hourly commitment on a weekly or bi-weekly basis
Key Responsibilities:
- Conduct comprehensive home health risk assessments for Medicare Advantage plan members
- Review and document prior/current medical histories, medical conditions, and medication usage
- Deliver clear patient education on chronic and acute conditions as well as medication regimens
- Identify potential care gaps and provide actionable recommendations for follow-up
- No requirement to prescribe medications or develop new treatment plans
Qualifications:
- Active Nurse Practitioner license in Tennessee
- Current Basic Life Support (BLS/CPR) certification
- Board Certification through AANP or ANCC
- Master of Science in Nursing (MSN) from an accredited institution
- At least six months current experience as a Nurse Practitioner preferred
This per diem role offers highly competitive compensation, with $80 to $90 per completed evaluation, plus additional mileage and administrative time stipends.
