Take on an impactful opportunity providing in-home care to patients as a Nurse Practitioner specializing in home health. Support Medicare Advantage plan members residing throughout Pittsburg County by performing comprehensive home health risk assessments. This per diem position offers unmatched flexibilitycreate your own schedule with shifts available any day of the week and commit to a minimum number of weekly or bi-weekly hours that fit your lifestyle. Youll work throughout convenient locations across Pittsburg County, including Hartshone, Krebs, Quinton, and McAlester.
Responsibilities:
- Conduct annual and post-discharge home health risk assessments for Medicare Advantage members
- Assess and document current and previous medical history, chronic conditions, and medication use
- Provide patient education on health conditions and prescribed medications
- Identify gaps in care and recommend actions to ensure high-quality health management
- Visits take approximately 45 minutes each; focus is on assessment, not treatment planning or prescribing
Qualifications:
- Valid Nurse Practitioner license in the state of Oklahoma
- Board certification from ANCC or AANP
- CPR (BLS) certification required
- Master of Science in Nursing (MSN) from an accredited nursing program
- Minimum 6 months of recent Nurse Practitioner experience preferred
Compensation includes $80$90 per completed evaluation, plus a mileage stipend and payment for administrative time.
