Deliver high-impact care while enjoying the flexibility and autonomy of per diem work as a Nurse Practitioner specializing in home health. In this role, you will perform home health risk assessments for Medicare Advantage plan members within Pittsburg County, covering areas including McAlester, Hartshorne, Krebs, and Quinton. Each assessment averages 45 minutes and may involve annual health checkups or post-discharge visits, helping to ensure optimal patient outcomes throughout the community.

- Minimum weekly or bi-weekly hourly commitment required
Primary Responsibilities:
- Conduct comprehensive home health risk assessments for Medicare Advantage members
- Review medical and medication histories, and assess current conditions thoroughly
- Educate patients regarding health conditions and medications to support ongoing wellness
- Identify care gaps and recommend actionable steps to promote better health outcomes
- No prescribing of medications or creation of treatment plans necessary
Desired Qualifications:
- Nurse Practitioner License in the state of Oklahoma
- Current CPR certification (BLS)
- Board certification via AANP or ANCC
- Six months or more of recent Nurse Practitioner clinical experience preferred
- MSN from an accredited nursing program
