Job Description

Step into a dynamic per diem Nurse Practitioner opportunity focused on home health risk assessments within Desha County. This role centers on providing valuable care and insight for Medicare Advantage plan members through efficient, patient-centered assessments. Serve individuals across a variety of locations in Desha County, including Dumas, McGehee, and Arkansas City.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Self-scheduling with the ability to create your own workweek
  • Minimum weekly or bi-weekly hourly commitment ensures autonomy and balance

Responsibilities:

  • Conduct thorough home-based risk assessments, averaging approximately 45 minutes per visit
  • Review and evaluate patient medical history, existing health conditions, and medication use
  • Educate patients on their conditions and prescribed medications, ensuring they understand their care needs
  • Identify care gaps and provide recommendations to improve health outcomes
  • Visits may include annual wellness assessments or post-discharge follow-ups
  • Note: This position does not require developing treatment plans or prescribing medications

Qualifications:

  • Active Nurse Practitioner license in Arkansas
  • Current Basic Life Support (BLS) / CPR certification
  • Board Certification through AANP or ANCC
  • MSN degree from an accredited nursing program
  • At least 6 months of recent Nurse Practitioner experience preferred
