This per diem position involves conducting comprehensive home health risk assessments for Medicare Advantage plan participants throughout Marlboro County, including areas such as Bennettsville, McColl, and Clio. Schedule:
- Flexible shifts available Monday through Sunday
- Self-directed schedulingchoose shifts that fit your lifestyle
- Minimum weekly or bi-weekly hourly commitment
Responsibilities include:
- Conducting annual risk assessments in patients homes, approximately 45 minutes per evaluation
- Reviewing medical history, current conditions, and medication regimens for each patient
- Educating patients on their health status, chronic conditions, and medications
- Identifying potential gaps in care and providing actionable recommendations
- No requirement for developing treatment plans or prescribing medications
Desired Qualifications:
- Active Nurse Practitioner license in South Carolina
- Board Certification through AANP or ANCC
- MSN degree from an accredited institution
- At least 6 months of recent NP experience (preferred)
- Current Basic Life Support (BLS/CPR) certification
