This per diem role allows for significant flexibility, offering you the opportunity to create your own schedule with shifts available any day from Monday through Sunday. Assignments may take you to various locations within Darlington County, including Hartsville, Darlington, and Lamar. Evaluations are typically scheduled for approximately 45 minutes and may include annual assessments or post-discharge visits.

Schedule:

• Minimum hourly commitment required weekly or bi-weekly

Key Responsibilities:

• Perform annual home health risk assessments for Medicare Advantage plan members in their residences
• Review and document patients prior and current medical history, conditions, and medication use
• Provide educational guidance on health conditions and prescribed medications
• Identify gaps in care and recommend solutions to optimize patient outcomes
• No responsibility for developing treatment plans or prescribing medications

Qualifications:

• Current Nurse Practitioner license in South Carolina
• Board Certified by AANP or ANCC
• Current CPR (BLS) certification required
• MSN degree from an accredited nursing program
• At least 6 months of recent Nurse Practitioner experience is preferred
