Make a significant impact in patient care by delivering high-quality home health risk assessments in Craven County, NC, including the surrounding communities of New Bern, Havelock, Vanceboro, and Bridgeton. This per diem opportunity offers outstanding flexibility for Nurse Practitioners seeking to design their own schedule and support Medicare Advantage members through important annual assessments and post-discharge visits.

Schedule/Hours:

• Flexible shifts available any day of the week, Monday through Sunday
• Create your own schedule to allow for work-life balance
• Minimum weekly or bi-weekly hourly commitment

Responsibilities:

• Conduct comprehensive home health risk assessments for Medicare Advantage health plan members in residential settings
• Evaluate prior and current medical history, medical conditions, and medication history
• Educate patients regarding their medical conditions and medication regimens
• Identify gaps in care and provide actionable recommendations for resolution
• No treatment plan creation or medication prescribing required
• Assessments typically take approximately 45 minutes each

Qualifications:

• Active Nurse Practitioner license in North Carolina
• Current BLS/CPR certification
• Board certification through AANP or ANCC
• MSN from an accredited nursing program
• At least 6 months of current Nurse Practitioner experience preferred
