Take your Nurse Practitioner expertise beyond the clinic by performing meaningful, in-home health risk assessments for Medicare Advantage plan members in Craven County, NC. In this per diem opportunity, you will have the flexibility to create your own schedule and make a lasting impact through comprehensive patient assessments. Your visitseach approximately 45 minutesmay involve annual health assessments or post-discharge evaluations, serving members across New Bern and neighboring communities such as Havelock, Vanceboro, and Bridgeton.

Schedule:

• Flexible shifts available between Monday and Sunday
• Self-scheduling for maximum work-life balance
• Minimum weekly or bi-weekly hourly commitment required

Key Responsibilities:

• Conduct home health risk assessments for a Medicare Advantage health plan
• Evaluate prior and current medical history, existing conditions, and medication usage
• Educate members on their health status, conditions, and prescribed medications
• Identify gaps in care, provide recommendations, and document findings
• No need to create treatment plans or prescribe medications

Qualifications:

• Valid Nurse Practitioner license in North Carolina
• Current CPR certification (BLS)
• Board Certification via AANP or ANCC
• MSN from an accredited nursing school
• At least 6 months of current Nurse Practitioner experience is preferred

You will be compensated per completed evaluation, with typical earnings between $80-$90 per assessment, plus a mileage stipend and additional time for administrative tasks.
