This role involves visiting Medicare Advantage plan members across Craven County, including areas such as New Bern, Havelock, Vanceboro, and Bridgeton. As a Nurse Practitioner, you'll partner with members to evaluate their health needs in the comfort of their homes, making a meaningful impact on their healthcare journeys.
Schedule:

Key Responsibilities:
- Conduct annual home health risk assessments and post-discharge visits for Medicare Advantage members
- Review prior and current medical history, evaluate medical conditions, and assess medication usage
- Provide clear education to members on their health conditions and prescribed medications
- Identify gaps in care and suggest actionable recommendations for improved outcomes
- No responsibility for creating treatment plans or prescribing medicationsfocus on assessment and education
Qualifications:
- Valid Nurse Practitioner license in North Carolina
- Current BLS/CPR certification
- Board certification through AANP or ANCC
- Minimum 6 months of recent Nurse Practitioner experience preferred
- MSN degree from an accredited nursing school
