Job Description
Take your Nurse Practitioner expertise into the community by performing in-depth home health risk assessments on Medicare Advantage members across Campbell County, TN. This per diem role offers the flexibility to design your own schedule, working shifts between Monday and Sunday, with options to commit a minimum number of hours each week or on a bi-weekly basis.
Schedule:
- Flexible shifts available Monday through Sunday
- Opportunity to self-schedule
- Minimum weekly or bi-weekly hour commitment
Key Responsibilities:
- Complete annual health assessments and post-discharge visits in the patients home setting
- Evaluate prior and current medical history, ongoing medical conditions, and comprehensive medication history
- Provide education to patients regarding their medical conditions and medications
- Identify gaps in care, make recommendations for follow-up, and support patient health management
- No requirement for developing treatment plans or prescribing medications
Qualifications:
- Active Nurse Practitioner License in Tennessee
- Current BLS (CPR) certification
- Board certified through AANP or ANCC
- MSN degree from an accredited nursing program
- At least six months of current NP experience preferred
This position offers compensation of $80$90 per completed evaluation, along with a mileage stipend and paid administrative time.
