Deliver meaningful care by utilizing your Nurse Practitioner expertise in a per diem home health setting. In this essential role, you will complete focused home health risk assessments with adult Medicare Advantage members living throughout Campbell County, TN, including areas such as LaFollette, Jacksboro, and Jellico. This position allows you to tailor your work schedule, providing flexibility and autonomy over your caseload.
Schedule:
- Flexible shifts available between Monday and Sunday
- Self-managed schedulingdesign your own workdays
- Minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Complete thorough home health risk assessments within patient residences, averaging 45 minutes per visit
- Assess medical history, ongoing health conditions, and current medication use
- Deliver patient-centered education related to health concerns and medication management
- Identify gaps in care and provide actionable recommendations
- No requirement to create treatment plans or prescribe medications
Qualifications:
- Active Nurse Practitioner license in Tennessee
- Master of Science in Nursing (MSN) from an accredited program
- Board certification through AANP or ANCC
- Current CPR/BLS certification
- At least 6 months experience as a Nurse Practitioner strongly preferred
