As a Nurse Practitioner specializing in home health, youll contribute vital expertise to Medicare Advantage plan members living in Robeson County, including the communities of Lumberton, Red Springs, and Pembroke. This position allows for a highly flexible schedule, empowering you to create your own hours and balance your commitments.
Schedule:
- Flexible shifts available Monday through Sunday
- Self-scheduling: Design your own work calendar
- Choose your minimum weekly or bi-weekly commitment
Key Responsibilities:
- Perform annual risk assessments in patients homes, each taking about 45 minutes
- Assess patients medical and medication histories, including prior and current health conditions
- Deliver informative patient education on chronic disease management and medication use
- Recognize and document any gaps in care, offering actionable recommendations to care teams
- Conduct visits for annual health check-ins and post-discharge follow-ups
- No direct medication prescribing or treatment planning required
Qualifications:
- Active Nurse Practitioner license in North Carolina
- Board Certification through AANP or ANCC
- Six months' NP experience preferred
- MSN from an accredited program
- Current BLS (CPR) certification
Successful candidates will receive $80$90 per evaluation, with mileage and administrative time compensated, making this ideal for those interested in supplemental income or work-life flexibility.
