Healthcare professionals seeking a dynamic, patient-centered position will find this per diem Nurse Practitioner opportunity both rewarding and flexible. This role centers on conducting home health risk assessments with patients throughout Robeson County. As a Nurse Practitioner in this position, you will visit Medicare Advantage plan members in their homes to perform comprehensive assessments, which typically take about 45 minutes each. Locations can include communities such as Lumberton, Red Springs, and Pembroke, with assignments scheduled within these areas as needed.

Schedule:

• Flexible shifts available Monday through Sunday
• Ability to create your own schedule
• Minimum weekly or bi-weekly hourly commitments to fit your availability

Responsibilities:

• Conduct annual health risk assessments and post-discharge visits for Medicare Advantage members at their residences
• Review and evaluate medical history, ongoing medical conditions, and medication usage
• Educate patients on health conditions and proper medication management
• Identify potential gaps in care and recommend actions to improve outcomes
• No development of treatment plans or prescription authority is required

Qualifications:

• Active Nurse Practitioner license in North Carolina
• Current CPR (BLS) certification
• Board certification through AANP or ANCC
• MSN degree from an accredited nursing program
• At least six months of current experience as a Nurse Practitioner preferred

This position compensates between $80-$90 per evaluation, with additional mileage stipend and administrative time included, reflecting the value and expertise you bring to patient care.
