Job Description

Experience a dynamic per diem opportunity as a Nurse Practitioner specializing in home health risk assessments across Scotland County. In this role, you will meet Medicare Advantage plan members where they reside, including communities such as Laurinburg, Wagram, and Gibson. Each risk assessment typically requires approximately 45 minutes and may range from annual health evaluations to post-discharge visits. Enjoy the autonomy to create your own schedule, with flexible shifts available every day of the week and the option for minimum weekly or bi-weekly hourly commitments.

Key Responsibilities:

  • Perform thorough annual home health risk assessments for Medicare Advantage members
  • Review and evaluate prior/current medical history, conditions, and medication regimens
  • Offer patient education regarding chronic and acute medical conditions, as well as medications
  • Identify gaps in care, providing actionable recommendations for improved health outcomes
  • No need to develop treatment plans or prescribe medications

Qualifications:

  • Active Nurse Practitioner license in North Carolina
  • Current BLS (CPR) certification
  • Board Certified via AANP or ANCC
  • Minimum of 6 months of NP experience is preferred
  • Masters degree in nursing (MSN) from an accredited program
