Job Description
Advance your nursing practice by delivering essential home health assessments in Pulaski County. This per diem opportunity enables Nurse Practitioners to provide high-quality healthcare services to Medicare Advantage plan members by conducting risk assessments in their own homes. Assessments take approximately 45 minutes and can include annual evaluations or post-discharge visits. Candidates may be scheduled throughout Pulaski County, serving residents in Pulaski, Dublin, and nearby areas.
Schedule:
- Flexible shifts available Monday through Sunday
- Ability to design your own work schedule
- Minimum weekly or bi-weekly hourly commitment is required
Primary Responsibilities:
- Perform comprehensive annual risk assessments in member homes for a Medicare Advantage health plan
- Review and document prior/current medical history, chronic conditions, and medication regimens
- Educate members on health conditions and medication usage
- Detect gaps in care and advise on solutions to improve health outcomes
- No treatment planning or medication prescribing is involved
Qualifications:
- Active Nurse Practitioner license in Virginia
- MSN from an accredited nursing program
- Board certification (AANP or ANCC)
- CPR (BLS) certification required
- Six months or more of recent Nurse Practitioner experience is preferred
