Take on a vital role as an experienced Nurse Practitioner in a dynamic home health setting, providing risk assessments to Medicare Advantage plan members throughout Summit County, Ohio, including areas such as Akron, Cuyahoga Falls, Stow, and Barberton. With a per diem structure and highly flexible scheduling, this opportunity allows you to create your own shifts between Monday and Sunday, with the option of committing to a minimum weekly or bi-weekly hourly requirement.
Schedule:

Key Responsibilities:
- Conduct comprehensive annual home health risk assessments
- Evaluate both prior and current medical history, medical conditions, and medications
- Identify gaps in care and make recommendations for closing these gaps
- Deliver education to members about their medical conditions and medication regimens
- Visits may include annual assessments or post-discharge follow-ups
- No requirement to create treatment plans or prescribe medications
Qualifications:
- Active Nurse Practitioner License in Ohio
- Valid CPR certification (BLS)
- Board Certification through AANP or ANCC
- Minimum of 6 months current nurse practitioner experience preferred
- Master of Science in Nursing (MSN) from an accredited institution
