Advance your Nurse Practitioner career with a dynamic per diem opportunity focusing on home health care. This flexible role involves conducting comprehensive assessments for Medicare Advantage members residing throughout Stark County, including Massillon, North Canton, Canton, and Alliance. As a key healthcare provider, youll play a vital role performing home health risk assessmentseach visit lasting about 45 minutestailored to the needs of each individual, such as annual health reviews or post-discharge evaluations.
Schedule:
- Flexible shifts available Monday through Sunday
- Design your own work schedule
- Minimum weekly or bi-weekly hourly commitment
Responsibilities:
- Complete annual home health risk assessments for Medicare Advantage members in their homes
- Review and evaluate prior and current medical history, medical conditions, and medication usage
- Educate patients on their health conditions and prescribed medications
- Identify care gaps and make recommendations to improve overall wellness
- No treatment planning or medication prescribing required
Qualifications:
- Licensed Nurse Practitioner in Ohio
- Current BLS certification (CPR required)
- Board Certified via AANP or ANCC
- A minimum of six months experience as a Nurse Practitioner is preferred
- MSN degree from an accredited nursing program
This role offers competitive payment, with $80-$90 per evaluation, alongside a mileage stipend and compensation for administrative time.
