Enjoy a dynamic and flexible per diem opportunity delivering high-quality care as a Nurse Practitioner specializing in home health risk assessments. This role focuses on supporting Medicare Advantage members throughout St. Lawrence County, with home visits taking place in communities such as Massena, Ogdensburg, Canton, Potsdam, and surrounding areas. Adaptable scheduling options allow you to set your own shifts, with a requirement of a minimum weekly or bi-weekly hourly commitment.
Schedule:

Key Responsibilities:
- Conduct comprehensive home health risk assessments for Medicare Advantage plan members
- Assess medical, surgical, and medication history in detail
- Offer patient education regarding diagnosed conditions and current medications
- Highlight care gaps and suggest actionable recommendations for improved outcomes
- Visits may be prompted by annual health evaluations or post-hospital discharge needs
- No on-site prescribing or treatment planning requiredfocus is on assessment and education
Qualifications:
- Active Nurse Practitioner license in New York State
- Board certification through AANP or ANCC
- Current CPR certification (BLS required)
- Minimum of 6 months experience as an NP preferred
- Master of Science in Nursing (MSN) degree from an accredited institution
