This per diem opportunity offers exceptional flexibility, allowing you to design your own schedule while serving Medicare Advantage plan members throughout Hamilton County, including Cincinnati, Forest Park, and Norwood.
Schedule:
- Flexible shifts available Monday through Sunday
- Create your own schedule
- Minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Conduct comprehensive annual home health risk assessments for Medicare Advantage members in their homes
- Evaluate patients medical history, current conditions, and medication regimen
- Educate patients about their health conditions and prescribed medications
- Identify potential gaps in care and recommend solutions to promote better health outcomes
- Focus on assessment; treatment planning and prescription management are not required
Qualifications:
- Active Nurse Practitioner license in Ohio
- Current CPR certification (BLS)
- Board Certification through AANP or ANCC
- Master of Science in Nursing (MSN) from an accredited program
- Minimum of 6 months experience as an NP preferred
- Strong clinical assessment and communication skills
