Step into an impactful advanced practice role providing home-based risk assessments for Medicare Advantage plan members across Blount County. As a Nurse Practitioner specializing in home health, youll connect directly with patients in Maryville, Alcoa, Friendsville, and surrounding areasdelivering crucial annual health and post-discharge assessments that often take just 45 minutes. This per diem opportunity gives you exceptional flexibility to tailor your schedule according to your lifestyle.
Schedule:
- Flexible shifts available Monday through Sunday
- Design your own work schedule
- Commit to a minimum number of hours weekly or bi-weekly to maintain your per diem status
Key Responsibilities:
- Perform in-home health risk assessments and document comprehensive findings
- Review past and current medical history, clinical conditions, and medications
- Provide clear patient education regarding health conditions and medication regimens
- Identify care gaps and make evidence-based recommendations to address them
- No requirement to prescribe medications or develop treatment plans
Qualifications:
- Active Nurse Practitioner license in Tennessee
- BLS/CPR certification
- Board certified through AANP or ANCC
- Minimum of 6 months recent NP experience strongly preferred
- Masters in Nursing (MSN) from an accredited institution
