An excellent opportunity awaits Nurse Practitioners skilled in home health risk assessments, looking for a per diem role with a flexible schedule. In this position, you will provide care and support to Medicare Advantage plan members in various communities within Yancey County. The work involves traveling to member residences for thorough health assessments, which may include annual health reviews or post-discharge visits, ensuring a broad impact on patient care.
Schedule:
- Flexible shifts available Monday through Sunday
- Ability to design your own schedule
- Choose your preferred weekly or bi-weekly hourly commitment
Key Responsibilities:
- Conduct home health risk assessments, typically 45 minutes per visit
- Review and evaluate current and past medical history, conditions, and medications
- Educate patients about their chronic conditions and prescribed treatments
- Identify care gaps and recommend next steps for follow-up
- Focus solely on assessmentsno requirements for developing treatment plans or prescribing medications
Qualifications:
- Active Nurse Practitioner license in North Carolina
- Current BLS/CPR certification
- Board Certified via AANP or ANCC
- Minimum six months of NP experience preferred
- Master of Science in Nursing (MSN) from an accredited institution
