Make a meaningful impact in community healthcare by leveraging your clinical expertise in a flexible, per diem capacity. The ideal candidate will work directly with Medicare Advantage members, conducting home health risk assessments in Mars Hill and surrounding areas of Madison County, NC. Each assessment typically takes about 45 minutes, varying by visit type, such as annual health evaluations or post-discharge reviews. Youll play a key role in patient education and care gap identification, without the need for prescribing medications or creating treatment plans.

- Flexible shifts available any day of the week (Monday through Sunday)

- Minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Perform comprehensive health risk assessments for Medicare Advantage plan members in their homes
- Review and assess current and prior medical history, conditions, and medications
- Provide patient education regarding diagnoses, medication management, and wellness
- Identify gaps in care and recommend interventions as appropriate
- Document findings clearly and accurately; follow up as needed
Qualifications:
- Active Nurse Practitioner license in North Carolina
- Board certification through AANP or ANCC
- Current CPR (BLS) certification
- MSN from an accredited nursing program
- A minimum of 6 months experience as a Nurse Practitioner is preferred
