Deliver vital healthcare services directly to patients in their homes by providing comprehensive home health risk assessments for Medicare Advantage plan members. This per diem opportunity offers you the flexibility to design your own schedule, working in areas throughout Camden County, including Camden, Cherry Hill, and Gloucester City. Each home visit typically takes around 45 minutes and could involve annual assessments or post-discharge evaluations, allowing you to focus on patient education and closing care gaps without the responsibility of creating treatment plans or prescribing medications.

Schedule:

• Flexible shifts available Monday through Sunday
• Freedom to create your own schedule
• Options for minimum weekly or bi-weekly hourly commitments

Primary Responsibilities:

• Perform annual risk assessments in home settings for Medicare Advantage members
• Thoroughly review and evaluate patients medical history, conditions, and medication usage
• Educate individuals about their medical conditions and medication management
• Detect any gaps in care and offer actionable recommendations to resolve them
• No medication prescribing or treatment planning required

Desired Qualifications:

• Active Nurse Practitioner license in NJ
• Current CPR certification (BLS)
• Board certification through AANP or ANCC required
• At least six months of recent Nurse Practitioner experience preferred
• MSN degree from an accredited nursing program
