This role focuses on conducting comprehensive home health risk assessments for Medicare Advantage plan members living in various areas across McDowell County, including Marion and Old Fort. Each visit is tailored to the individual, ranging from annual health assessments to post-discharge evaluations. Average assessments take about 45 minutes, and your advanced clinical expertise will play a key role in optimizing patient outcomes and supporting preventive care.

Schedule:

• Flexible shifts available Monday through Sunday
• Design your own work schedule
• Commit to weekly or bi-weekly hourly minimums to suit your availability

Key Responsibilities:

• Perform detailed annual home health risk assessments for a Medicare Advantage health plan
• Thoroughly review medical and medication history; evaluate current and past health concerns
• Educate patients on their medical conditions and prescribed medications
• Identify care gaps and recommend next steps for resolution
• No treatment plan development or medication prescribing required

Qualifications:

• Nurse Practitioner licensure in North Carolina
• Current CPR certification (BLS)
• Board Certification via AANP or ANCC
• Completion of an MSN degree from an accredited nursing school
• At least 6 months of current Nurse Practitioner experience preferred

This opportunity offers compensation of $80$90 per assessment, along with a mileage stipend and payment for administrative time.
