This per diem opportunity is tailored for skilled Nurse Practitioners with a focus on home health services. You'll provide high-impact care by conducting risk assessments for Medicare Advantage plan members in Burke County, NC. Locations may include Morganton, Hickory, Valdese, and nearby areas; each home risk assessment typically lasts about 45 minutes and may involve annual health evaluations or post-discharge visits.
Schedule:
- Flexible shifts available Monday through Sunday
- Choose your own workdays
- Minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Perform comprehensive home health risk assessments for Medicare Advantage plan members.
- Review and evaluate prior and current medical history, active medical conditions, and medication usage.
- Educate patients on their health conditions and medication regimens, ensuring clear understanding.
- Identify and communicate gaps in care, offering recommendations to improve overall patient health.
- No requirement to create treatment plans or prescribe medications.
Qualifications:
- Active Nurse Practitioner License in North Carolina
- Current BLS (CPR) certification
- Board certified through AANP or ANCC
- At least 6 months of recent experience as a Nurse Practitioner preferred
- MSN degree from an accredited nursing school
Compensation and Benefits: Earn $80$90 per completed evaluation, and receive a mileage stipend plus compensation for administrative time.
