This per diem opportunity is tailored for skilled Nurse Practitioners with a focus on home health services. You'll provide high-impact care by conducting risk assessments for Medicare Advantage plan members in Burke County, NC. Locations may include Morganton, Hickory, Valdese, and nearby areas; each home risk assessment typically lasts about 45 minutes and may involve annual health evaluations or post-discharge visits.

Schedule:

• Flexible shifts available Monday through Sunday
• Choose your own workdays
• Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

• Perform comprehensive home health risk assessments for Medicare Advantage plan members.
• Review and evaluate prior and current medical history, active medical conditions, and medication usage.
• Educate patients on their health conditions and medication regimens, ensuring clear understanding.
• Identify and communicate gaps in care, offering recommendations to improve overall patient health.
• No requirement to create treatment plans or prescribe medications.

Qualifications:

• Active Nurse Practitioner License in North Carolina
• Current BLS (CPR) certification
• Board certified through AANP or ANCC
• At least 6 months of recent experience as a Nurse Practitioner preferred
• MSN degree from an accredited nursing school

Compensation and Benefits: Earn $80$90 per completed evaluation, and receive a mileage stipend plus compensation for administrative time.
