Job Description

Nurse Practitioners with home health experience have a unique opportunity to make a meaningful impact by conducting risk assessments in a per diem role. This position supports Medicare Advantage plan members residing in Marion and nearby areas of Grant County, ensuring high-quality care through flexible, community-based visits.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Create your own schedule
  • Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

  • Conduct comprehensive annual home health risk assessments for Medicare Advantage members during 45-minute visits in patients homes
  • Assess past and current medical histories, chronic conditions, and medication use
  • Provide patient education on health conditions and medications
  • Identify and address care gaps, making evidence-based recommendations
  • Each visit may include annual health assessments or post-discharge follow-ups
  • No requirement to develop treatment plans or prescribe medications

Qualifications:

  • Active Nurse Practitioner (NP) license in the state of Indiana
  • Current BLS/CPR certification
  • Board Certification as an NP through AANP or ANCC
  • MSN degree from an accredited nursing program
  • At least 6 months of recent NP experience preferred, especially in home health

In addition to the professional satisfaction of working independently and directly impacting patient outcomes, you can expect earnings of $80$90 per visit completed, plus a mileage stipend to cover travel and administrative time.
