Deliver high-quality patient care while enjoying the flexibility of per diem work as a Nurse Practitioner conducting home health risk assessments. This opportunity covers Cedar Rapids, Marion, Hiawatha, and surrounding communities throughout Linn County. Candidates will engage with Medicare Advantage members in their homes, making a meaningful impact on patient well-being through comprehensive assessments and education.

Schedule:

• Flexible shifts offered Monday through Sunday
• Design your own schedule to meet your availability
• Minimum weekly or bi-weekly hourly commitment

Key Responsibilities

• Perform annual home health risk assessments for Medicare Advantage plan participants
• Review current and prior medical history, chronic conditions, and medications
• Educate members on health conditions, medication adherence, and self-care
• Detect care gaps and make informed recommendations for follow-up
• No requirement to develop treatment plans or prescribe medication

Qualifications:

• Active Nurse Practitioner license in IA
• Current CPR (BLS) certification
• Board Certification from AANP or ANCC
• At least 6 months of recent nurse practitioner experience preferred
• Master of Science in Nursing (MSN) from an accredited program

Youll receive $80$90 per evaluation and a mileage stipend, plus compensation for administrative time. This role allows you to focus on assessment, education, and care improvementwithout the need for ongoing patient management or prescribing responsibilities.
