Step into a rewarding per diem opportunity as a Nurse Practitioner specializing in home health risk assessments for Medicare Advantage plan members residing in Jackson County, FL. This position focuses on comprehensive, in-home evaluations designed to improve care and enhance patient outcomes across Marianna, Graceville, Sneads, and surrounding communities.
Schedule:
- Enjoy flexible shifts Monday through Sunday.
- Design your own schedule to best suit your lifestyle.
- Commit to a minimum number of hours per week or bi-weekly, based on your availability.
Key Responsibilities:
- Conduct annual risk assessments in patient homes, averaging 45 minutes per visit.
- Collect and review prior and current medical histories, chronic conditions, and medication lists.
- Deliver patient education regarding medical conditions and prescribed medications.
- Identify care gaps and offer recommendations for resolution.
- No requirement to create treatment plans or prescribe medicationsfocus strictly on assessment and education.
Qualifications:
- Active Nurse Practitioner license in the state of Florida.
- Valid CPR certification (BLS).
- Board Certification through AANP or ANCC.
- At least six months of current experience as a Nurse Practitioner preferred.
- MSN from an accredited nursing program.
For each completed evaluation, you will receive competitive pay ranging from $80 to $90, in addition to compensation for mileage and administrative time.
