Exciting opportunity for a Nurse Practitioner with experience in home health to perform in-depth risk assessments for Medicare Advantage members in St. Francis County, AR. This per diem position offers maximum flexibility, allowing you to build a schedule that fits your lifestyle, with shifts available throughout the weekincluding weekends. You'll perform comprehensive health evaluations for members in their homes, focusing on overall wellness and preventive care rather than prescribing or creating treatment plans.
Schedule:
- Flexible shifts offered Monday through Sunday
- Create your own schedule
- Choose minimum weekly or bi-weekly hourly commitments
Key Responsibilities:
- Conduct annual health risk assessments in patient homes for Medicare Advantage health plan participants
- Review and evaluate prior and current medical, surgical, and medication histories
- Educate members about their conditions, medications, and general health maintenance
- Identify care gaps and provide actionable recommendations to help close them
- No requirement to prescribe medications or manage long-term treatment
Preferred Qualifications:
- Active Nurse Practitioner license in Arkansas
- MSN degree from an accredited program
- Board certification through AANP or ANCC
- Current CPR (BLS) certification
- Minimum of 6 months NP experience preferred, especially in home health or similar settings
