Advance your career as a Nurse Practitioner specializing in home health care, performing risk assessments for Medicare Advantage plan members in the Pike County, AR area, including Glenwood, Murfreesboro, and Delight. This per diem opportunity offers autonomy, flexibility, and the ability to create your own work scheduleperfect for balancing professional responsibilities with personal commitments.
Schedule:
- Flexible shifts available Monday through Sunday
- Self-scheduling to suit your availability
- Minimum weekly or bi-weekly hourly commitment required
Key Responsibilities:
- Perform comprehensive home health risk assessments, averaging 45 minutes each, for Medicare Advantage members
- Evaluate patients medical history, current medical conditions, and medication profiles
- Deliver patient education on managing chronic and acute conditions as well as proper medication usage
- Identify any gaps in care and recommend solutions to improve overall health outcomes
- No prescribing of medications or development of treatment plans required
Qualifications:
- Valid Nurse Practitioner license in Arkansas
- Current BLS/CPR certification
- Board certification through AANP or ANCC
- Minimum of 6 months of current experience as a Nurse Practitioner preferred
- Master of Science in Nursing (MSN) degree from an accredited institution
