Make an impact on community health by performing home-based risk assessments for Medicare Advantage members in Lee County, AR, including Marianna, Moro, and Rondo. In this dynamic per diem role, youll work independently as a Nurse Practitioner, visiting patients in their homes to assess health status, identify care gaps, and foster patient education. Each home health risk assessment typically takes about 45 minutes, and assignments may include annual wellness checks or post-discharge evaluations.
Schedule:
- Flexible hours available Monday through Sunday
- Choose your own shifts
- Minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Complete annual home health risk assessments for Medicare Advantage plan participants
- Review previous and current medical conditions, medications, and comprehensive health history
- Educate members on disease management, preventive care, and proper medication usage
- Identify any existing gaps in care and recommend appropriate follow-up actions
- No treatment planning or medication prescribing required
Qualifications:
- Active Nurse Practitioner License in Arkansas
- Current BLS (CPR) certification
- Board Certification through AANP or ANCC
- MSN degree from an accredited nursing program
- At least 6 months of recent NP experience preferred
