Job Description
Deliver quality care in a home health setting as a Nurse Practitioner, supporting Medicare Advantage members across Collier County, FLincluding Naples, Immokalee, and Marco Island. This per diem position offers a highly flexible schedule, allowing you to create your own shifts between Monday and Sunday, with the opportunity to commit to a minimum number of hours weekly or bi-weekly.
Schedule:
- Flexible shifts available Monday through Sunday

- Minimum weekly or bi-weekly hourly commitment required
Responsibilities:
- Conduct home-based health risk assessments, focusing on Medicare Advantage health plan members.
- Perform thorough reviews of current and previous medical history, including evaluation of medical conditions and medication regimens.
- Educate patients on their diagnoses and prescribed medications.
- Identify gaps in care and make informed recommendations to ensure enhanced patient outcomes.
- Visits typically last approximately 45 minutes and may be annual health assessments or post-discharge visits.
- There is no requirement to create treatment plans or prescribe medications.
Desired Qualifications:
- Active Nurse Practitioner License in Florida.
- Board certified through either AANP or ANCC.
- Current CPR certification (BLS).
- At least 6 months of recent Nurse Practitioner experience preferred.
- MSN degree from an accredited nursing program.
This role provides compensation of $80$90 per evaluation, along with mileage reimbursement and additional compensation for administrative time.
