Deliver meaningful, patient-centered care as a skilled Nurse Practitioner conducting home health risk assessments for Medicare Advantage members. This per diem opportunity is ideal for practitioners who value flexibility and self-directed scheduling while making a tangible impact in the community.

Schedule:

Key Responsibilities:

• Complete comprehensive annual home health risk assessments for Medicare Advantage plan members within the community
• Assess prior and current medical history, ongoing conditions, and current medications
• Deliver patient education regarding medical issues and medication regimens
• Identify care gaps and recommend solutions to support optimal health outcomes
• Focus exclusively on assessmentno treatment plans or prescribing required

Qualifications:

• Active Nurse Practitioner License in Missouri
• MSN degree from an accredited nursing program
• Board Certification via AANP or ANCC
• Basic Life Support (BLS/CPR) certification
• At least 6 months of nurse practitioner experience preferred

This position offers $80$90 for each completed evaluation, with additional mileage and administrative stipends to support your time and travel. You will work within the communities of Marble Hill and greater Bollinger County, including Lutesville, Zalma, and Glenallen. Each assessment takes approximately 45 minutes, allowing you to manage your time efficiently throughout each shift.
