Deliver essential healthcare to individuals in their homes as a Nurse Practitioner providing home health risk assessments. This per diem role focuses on performing thorough evaluations for Medicare Advantage plan members in Marble Falls and the greater Burnet County area. Each visit lasts approximately 45 minutes and may serve as an annual assessment or post-discharge check-in. Flexibility is central to this positionyou control your schedule, choosing shifts any day of the week and determining your own commitment level.

Schedule:

• Flexible shifts available Monday through Sunday
• Set your own schedule
• Meet hourly minimums weekly or bi-weekly

Key Responsibilities:

• Perform comprehensive home health risk assessments for Medicare Advantage plan members
• Review patients past and current medical history, conditions, and medication regimens
• Provide guidance and education on chronic and acute health conditions, as well as prescribed medications
• Identify care gaps and offer actionable recommendations
• No prescribing medications or developing treatment plans required

Qualifications:

• Active Nurse Practitioner licensure in Texas
• Current CPR certification (BLS)
• National Board Certification (AANP or ANCC)
• Minimum of 6 months recent NP experience preferred
• Master of Science in Nursing (MSN) from an accredited program
