Deliver direct care impact by performing home health risk assessments for Medicare Advantage plan members throughout Aroostook County. As a Nurse Practitioner specializing in Home Health, you'll engage with patients in their residences, focusing on thorough, patient-centered evaluations. Schedule:

• Flexible shifts available Monday through Sunday
• Option to customize your own schedule
• Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

• Conduct in-home risk assessments for Medicare Advantage enrollees
• Review patients medical history, current health status, and medication regimens
• Offer patient education concerning health conditions and prescribed medications
• Identify care gaps and recommend actionable solutions to enhance wellbeing
• Visits may involve annual assessments or post-discharge check-ins
• No requirement to develop treatment plans or prescribe medications

Qualifications:

• Active Nurse Practitioner license in the state of Maine
• Board certification through AANP or ANCC
• Current CPR certification (BLS)
• Masters degree in Nursing (MSN) from an accredited institution
• At least 6 months of recent nurse practitioner experience preferred
