Take on a rewarding per diem position conducting home health risk assessments throughout Otero County, Colorado. In this role, you will utilize your Nurse Practitioner expertise to complete focused, 45-minute visits with Medicare Advantage plan members in their homes. Your day-to-day could include appointments in communities such as La Junta, Rocky Ford, or Fowler.
Schedule:
- Flexibility to choose shifts Monday through Sunday
- Create your own schedule to fit your availability
- Choose minimum weekly or bi-weekly hourly commitments
Responsibilities:
- Perform comprehensive annual home health risk assessments for Medicare Advantage members
- Review and evaluate prior/current medical history, chronic conditions, and medication use
- Provide patient education on health conditions and medication management
- Identify any care gaps and recommend next steps for the member's healthcare team
- No requirements for prescribing or treatment plan creation
Qualifications:
- Active Nurse Practitioner license in Colorado
- Current BLS CPR certification
- Board Certified through AANP or ANCC
- Minimum of 6 months experience as a Nurse Practitioner preferred
- MSN degree from an accredited nursing program
