This per diem opportunity is ideal for skilled professionals seeking flexibility and a chance to serve Medicare Advantage plan members through comprehensive risk assessments. As a vital part of this initiative, you will travel throughout areas such as Kill Devil Hills, Atlantic township, Nags Head township, and Hatteras township, delivering high-quality, patient-centered care in clients homes.
Schedule:
- Flexible shifts available Monday through Sunday
- Create your own work schedule
- Minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Conduct thorough annual home health risk assessments for Medicare Advantage members
- Evaluate a patients medical history, current health conditions, and medication usage
- Educate patients on health conditions and medication management
- Identify gaps in care and issue practical recommendations
- Visits do not require you to prescribe medication or develop treatment plans
- Document findings according to guidelines and uphold patient privacy standards
Qualifications:
- Active Nurse Practitioner license in North Carolina
- Board certification through AANP or ANCC
- MSN from an accredited nursing program
- Current BLS (CPR) certification
- At least 6 months recent Nurse Practitioner experience preferred
- Strong communication and interpersonal skills
- Reliable transportation to travel within Dare County
Those in this role will receive $80$90 per completed evaluation, in addition to a mileage stipend and compensation for administrative time.
