In this per diem position, youll take on meaningful work by conducting risk assessments for Medicare Advantage plan members throughout Richland County, Ohio, including communities such as Mansfield, Ontario, and Madison Township. Enjoy the flexibility to craft your own schedule and commit to a minimum number of hours per week or every two weeks, making it ideal for those seeking autonomy and balance.
Schedule:
- Flexible shifts available between Monday and Sunday
- Design your own work schedule
- Minimum weekly or bi-weekly hourly commitment
Responsibilities:
- Perform annual home health risk assessments for Medicare Advantage members in the community
- Evaluate a patients prior and current medical history, ongoing conditions, and medication regimens
- Deliver clear education on health conditions and prescribed medications
- Identify gaps in care and suggest actionable recommendations to enhance patient well-being
- Note: No requirement to create treatment plans or prescribe medications
Qualifications:
- Active Nurse Practitioner license in Ohio
- Current CPR certification (BLS)
- Board certification via AANP or ANCC
- Preferred minimum of six months of current NP experience
- MSN degree from an accredited nursing program
This position offers compensation at $80$90 per completed evaluation, in addition to a mileage stipend and paid administrative time, making your expertise and dedication truly worthwhile.
