Job Description

As a Nurse Practitioner specializing in home health, you'll play a vital role in supporting patient wellness by performing risk assessments for Medicare Advantage plan members across Onondaga County. This per diem opportunity offers maximum flexibility, allowing you to tailor your work hours based on your availability while making a significant impact on community health.

Schedule:

  • Flexible shifts available seven days a week
  • Create your own schedule based on your preferences
  • Minimum weekly or bi-weekly hourly commitment required

Key Responsibilities:

  • Complete comprehensive home health risk assessments (approximately 45 minutes per visit)
  • Review medical histories, current conditions, and medication use
  • Deliver patient education regarding their health conditions and prescribed medications
  • Identify care gaps and recommend appropriate resolutions
  • Visits may include annual assessments or post-discharge follow-ups
  • No requirement to create treatment plans or prescribe medications

Qualifications:

  • Active Nurse Practitioner license in New York State
  • Current CPR (BLS) certification
  • Board Certification via AANP or ANCC
  • Minimum of 6 months recent NP experience preferred
  • Master of Science in Nursing (MSN) degree from an accredited institution

In return for your clinical expertise, youll receive compensation of $80-$90 per completed evaluation, in addition to a mileage stipend and pay for administrative time. This role provides ample autonomy and the rewarding experience of one-on-one patient interaction in their home environment.
