Deliver quality care on your own terms as a Nurse Practitioner specializing in Home Health assessments throughout Onondaga County. This per diem position allows you to create your own flexible schedule with shifts available Monday through Sunday and only a minimal weekly or bi-weekly commitment required.

Schedule:

• Flexible shifts between Monday and Sunday
• Choose your own schedule
• Minimum weekly or bi-weekly hourly commitment

In this role, you will perform comprehensive home health risk assessments for Medicare Advantage plan members. Each visit averages 45 minutes and may include annual health assessments or post-discharge evaluations. Your assignments could be in any area within Onondaga County, including Syracuse, Salina, Onondaga, and Clay.

Key Responsibilities:

• Complete annual home health risk assessments for Medicare Advantage health plans
• Thoroughly review prior and current medical history, conditions, and medication usage
• Educate members about their medical conditions and medications
• Identify care gaps and provide actionable recommendations for improvement
• No medication prescribing or treatment plan development is required

Qualifications:

• Active Nurse Practitioner license in NY
• Current CPR certification (BLS)
• Board Certification via AANP or ANCC
• MSN degree from an accredited nursing school
• At least 6 months of recent Nurse Practitioner experience is preferred

This position offers $80$90 per evaluation, along with mileage and administrative time compensation.
