Make a meaningful impact in patient care by performing home health risk assessments within Onondaga County, NY. This per diem opportunity is ideal for a Nurse Practitioner with a passion for home health and patient-centered assessments. visits are conducted across various locations, including Syracuse, Salina, Onondaga, and Clay, with potential for both annual and post-discharge evaluations focused on Medicare Advantage plan members.
Schedule:
- Flexible shifts available Monday through Sunday
- Build your own schedule
- Choose a minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Complete comprehensive annual risk assessments in patients homes
- Review medical history, evaluate current conditions, and document medication use
- Educate patients on diagnoses and prescribed therapies
- Identify gaps in care and recommend next steps for resolution
- No need to create detailed treatment plans or prescribe medications
Qualifications:
- Active Nurse Practitioner License in NY State
- Board Certification through AANP or ANCC
- Current CPR certification (BLS)
- MSN degree from an accredited nursing program
- At least 6 months of recent Nurse Practitioner experience preferred
Compensation for this role includes a per-visit payment of $80$90 per evaluation, in addition to a mileage stipend and time allotted for administrative documentation.
