This per diem opportunity is designed for skilled Nurse Practitioners who are passionate about community-focused healthcare, offering the flexibility to create a personalized work schedule. The practitioner will conduct comprehensive home health risk assessments for Medicare Advantage plan members throughout Onondaga County, including areas such as Salina, Onondaga, Clay, and Syracuse. Each home visit averages 45 minutes and may encompass annual health reviews or post-discharge follow-ups, with the goal of improving patient outcomes and supporting coordinated care.

Schedule:

• Flexible shifts available Monday through Sunday
• Option to design your own schedule to fit personal and professional needs
• Commit to minimum weekly or bi-weekly hours as preferred

Key Responsibilities:

• Complete annual risk assessments in patient homes in accordance with Medicare Advantage standards
• Conduct thorough reviews of current and prior medical histories, as well as current health conditions and medications
• Educate patients about their conditions and prescribed treatments to promote understanding and adherence
• Recognize gaps in patient care and provide practical recommendations to address them
• No prescriptive authority or treatment plan creation required

Qualifications:

• Current, active Nurse Practitioner license in NY State
• Board Certification through AANP or ANCC
• Active CPR certification (BLS)
• MSN from an accredited nursing school
• At least 6 months of recent Nurse Practitioner experience preferred
